Check Request Submission
Completed check request forms are required by all members when seeking reimbursement of expenses. The form and supporting documentation with receipts should sent to the SAR San Diego Treasurer, Daniel McLaughlin. Be sure to sign your check request before sending to the president.
Click here to download and complete the fill-in Acrobat form necessary to request expense reimbursement from the chapter. Use the instructions below when completing the form. When completed you may print it and submit to the Chapter Treasurer, Dick Leslie at email@example.com.
